Here you can find various forms utilized by the city and it's corresponding departments.  There are two methods of submitting these forms. You can click on the icon in the download column which will download a PDF form.  You can enter the information in the form, print it, and submit it to the relevant department.  *You will need a PDF reader in order to do this.  The most common is the free Adobe Acrobat Reader.  Many modern browsers also have the ability to open these forms natively. 

The second option is if a form has an Online submission option, you can click on the icon in the Online column which will take you to the online form. You will fill out all required forms and click submit. The City office staff will receive your submission and contact you will any follow-up information or questions they may have.

Specials Notes:

• Not all of our forms have an online submission option.  If they do there will be an icon in the online column.
• Forms that have Personal Identification Information will have those fields on the Online Submission forms, but you cannot enter those fields for security reasons.  Go ahead and submit the form with all the other information, and a city staff member will contact you to get this information as needed.
• Some of these applications require fees to be paid prior to them being processed.  Please check with the department handling the form on its requirements for final approval.
